Student Feedback Form

This form allows you to give feedback on tutorials with Queen’s College and external tutors. Please complete a separate form for each of the tutors about whom you would like to give feedback. You are not obliged to give your name, but you are encouraged to do so, so that the Senior Tutor can respond to, or enquire further about, matters of concern. The form will not be shown to the subject tutor unless you indicate in the mandatory field at the bottom of the form that you are happy for him or her to see it. Forms about tutorials with the Senior Tutor will be sent to the Provost.
